Application Process:
Consult Your Healthcare Provider: Discuss your eligibility with your healthcare provider and obtain a recommendation.
Submit Your Application: Your healthcare provider will verify your need and submit the recommendation to The Crowns Project.
Receive Your Supplies: Once approved, you will receive a three-month supply of incontinence undergarments.
Referral Application Form: To apply for the Goal Assist program, please complete the referral application form provided by your healthcare provider.
